Abstract:OBJECTIVE To explore the value of community family doctors taking HbA1c as a clue to actively monitor the blood glucose control of diabetic patients in outpatient clinics, and combining"all-specialty combination"outpatient clinics in diabetic patients.METHODS The family doctors in our center actively monitored diabetic Patients with HbA1c≥7%in the outpatient clinic from August 1st,2022 to June 30th,2023.The patients were divided into general management group(n=203)and control group(n=122)according to whether they were treated in the"all-specialty combination"outpatient department of diabetes. The differences of blood glucose control indexes were compared between the two groups before intervention and after 3 months of intervention.RESULTS There was no significant difference in age, gender, course of disease, waist circumference, hip circumference,BMI,fasting blood glucose, blood glucose 2 hours after meals, blood glucose before going to bed and HbA1c between the comprehensive management group and the control group before intervention(P>0.05).After 3 months of intervention, the blood glucose levels 2 hours after meals, blood glucose levels before bed and HbA1c in the comprehensive management group were lower than those in the control group, with statistical significance(t=-3.440,-2.040,-1.980,P<0.05).There was no significant difference in fasting blood glucose between the two groups(P>0.05).CONCLUSION Compared with the routine outpatient treatment, the"warning value"combined with"general and special combination"management mode of HbA1c is more conducive to the control of blood glucose and HbA1c in community diabetic Patients, and has good application value in community diabetes management.
