Reliability and validity study of the NASA-TLX scale for assessing residents'job stress during emergency department residency training in a tertiary hospital and analysis of associated stressors

Abstract:Objective To test the reliability and validity of the NASA-TLX scale for assessing the work stress of residents in tertiary hospitals during emergency department residency training and to analyse the associated stressors so as to provide a tool for assessing the work stress of residents during emergency department residency training.Methods A random sampling method was used to select 200 residents trained in the Emergency Department of The First Hospital of Jiaxing from April 2023 to May 2024 for the survey.General information questionnaire and NASA-TLX scale were used to investigate the study subjects.Internal consistency was used for reliability evaluation,structural validity was used for validity evaluation,and paired-sample t-test was used to compare the differences in work stress levels between residents before and after emergency department residency training.Multiple linear regression was used to analyse the effect of different factors on residents'work stress in the Emergency Department.Results NASA-TLX scale Cronbach's α coefficient was 0.632.The results of KMO test and Bartlett's spherical test showed KMO=0.696,P<0.05,which was consistent with the sphericity test.Through principal component analysis,a total of 2 common factors with feature roots>1.000 were extracted,with a cumulative variance contribution rate of 59.399%and load values of 0.569~0.930 for each item.Residents'work stress increased after training in the Emergency Department compared with before training[NASA-TLX scale score(70.64±7.01)versus(57.14±5.14),P<0.001].The multiple linear regression model showed that gender(β=-0.429,P<0.001),source of training residents(β=0.35,P<0.05)and seniority(β=-0.216,P<0.001)might predict NASA-TLX scores,while age and professional might not predict NASA-TLX scores(all P>0.05).Conclusion The NASA-TLX scale can be used to assess the work stress of tertiary hospital residents during emergency department training.The gender,seniority and source of training residents may be predictive of resident work stress during this period,with women,lower seniority and master's degree holders likely being more stressed.
