Abstract:Objective To explore the clinical and pathological characteristics and differential diagnosis of primary adult pulmonary fibrosarcoma (PAPFC) in an elderly female patient. Methods Retrospectively analyzed the histomorphology and immunophenotype of this case. Results A 75-year-old female patient was admitted for a left lower lung nodule found during a physical examination three weeks prior. Grossly, a gray-white nodular lesion was observed in the lower lobe of the left lung, well-demarcated from surrounding tissues, measuring 3.5 cm × 2.8 cm × 3.8 cm, 1 cm away from the bronchial margin, and close to the pleura. Microscopically: spindle cell tumor with focal hemorrhage. Immunohistochemistry: Vimentin positive expression, Ki-67 (20%), AE1/AE3 (-), S-100, SOX10, CD34, SMA, CD99, ALK, Bcl-2, Caldesmon negative. Conclusion Primary adult pulmonary fibrosarcoma is rare, and it is even rarer in elderly females. Its clinical symptoms and imaging findings are easily misdiagnosed. Through the report of this case and a review of the literature, we aim to enhance awareness of this disease among clinicians.
