Abstract:Objective: To explore the clinical pathological features and immunohistochemical phenotype of metastatic ovarian clear cell carcinoma (OCCC). Methods: A rare case of ovarian clear cell carcinoma metastasizing to the lung was reported, and its clinical data, histological characteristics, and immunohistochemistry were analyzed and discussed. Results: A 48-year-old female patient presented with cough and sputum, and chest CT showed a mass in the lower lobe of the right lung, measuring 4.7 cm × 3.8 cm × 2.8 cm. The cut surface was gray-white or gray-brown, solid, and medium in consistency. Microscopically, the tumor showed papillary and lepidic growth patterns, with polygonal tumor cells exhibiting a 'shoe钉-like' appearance, large nuclei, marked anaplasia, and eosinophilic or clear cytoplasm. Immunohistochemistry showed positivity for HNF-1β, NapsinA, PAX8, and CK7, and negativity for TTF-1. Based on the patient's medical history, the diagnosis was ovarian clear cell carcinoma with pulmonary metastasis. Conclusion: Metastatic ovarian clear cell carcinoma to the lung is rare. Its histological morphology can be easily misdiagnosed as primary pulmonary adenocarcinoma. A definitive diagnosis requires careful consideration of the patient's clinical history and immunohistochemical staining. Pathologists should maintain high vigilance when diagnosing metastatic tumors to avoid misdiagnosis.
