Abstract:This article reports a rare case of ovarian mesonephric-like carcinosarcoma. The patient underwent total hysterectomy with bilateral salpingo-oophorectomy due to a pelvic mass found during a physical examination. Pathological examination showed a cystic-solid tumor. Microscopically, the features were distinct: approximately 60% was mesonephric-like adenocarcinoma (with glandular/ papillary structures, expressing CK/TTF1/PAX8), and approximately 40% was high-grade sarcoma with rhabdomyosarcomatous differentiation (expressing Desmin/Myogenin). Immunohistochemistry clearly distinguished the carcinoma and sarcoma components. This case had a poor prognosis and provided detailed clinical pathological data for this extremely rare gynecological malignancy. Follow-up until June 1, 2025, showed recurrence and metastasis.
