Abstract:This study discusses the clinical and pathological characteristics, diagnosis, and differential diagnosis of gastric metastatic clear cell renal cell carcinoma. One case of gastric metastatic clear cell renal cell carcinoma diagnosed in our hospital in 2023 was collected and analyzed in combination with relevant literature. The patient was a 71-year-old female who presented with generalized weakness, decreased appetite, and dizziness and nausea. Endoscopic examination revealed destruction of the glandular structure in the body of the stomach, with infiltrative growth of cancer cells arranged in nests or acinar patterns. The stromal capillaries were abundant, and the nuclei showed mild atypia with visible nucleoli. The cytoplasm was clear, and some tumor cells had eosinophilic cytoplasm. No mitotic figures were observed. Immunophenotype: Cancer cells showed positive expression for AE1/AE3, PAX8, CD10, CAIX, and Vimentin, while CK7, CK20, TFE3, TFEB, E-cad, CD117, P63, CD68, α-inhibin, Syn, CgA, and HMB45 were negative. The Ki-67 proliferation index was 10%. Gastric metastasis of clear cell renal cell carcinoma is a rare condition. This tumor needs to be differentiated from primary and other secondary tumors with clear cell morphology in the stomach. A definitive diagnosis can be made by combining the patient's medical history and immunohistochemical staining, thus avoiding missed or misdiagnosis.
