Clinicopathological features of primary SMARCA4-deficient sinonasal carcinoma

Abstract:Objective To summarize the clinical and pathological features of primary SMARCA4-deficient nasal and paranasal sinus carcinoma. Methods Clinical data of a patient with primary SMARCA4-deficient nasal and paranasal sinus carcinoma were collected. The clinical and pathological characteristics were analyzed by H&E staining, immunohistochemistry, and molecular testing. Results Under low magnification: tumor cells infiltrated within the mucosa and submucosa, arranged in nests and diffuse solid sheets, with necrosis, no glandular, squamous, or other differentiation features. Under high magnification: the tumor showed a syncytial-like appearance, with high nuclear-to-cytoplasmic ratio and prominent nucleoli. Immunohistochemical results: tumor cells expressed CKpan, CAM5.2, Syn, and INI-1, but not BRG1, P63, NUT, Desmin, LCA, or S100. Molecular testing: ERER(-). Conclusion Primary SMARCA4-deficient nasal and paranasal sinus carcinoma is rare, with non-specific pathological morphology, which is easily misdiagnosed. Timely and accurate diagnosis has important guiding significance for the treatment and prognosis of patients.
