EBV-positive diffuse large B-cell lymphoma:report of a case

Abstract:Objective To investigate the clinical pathological features and immunophenotype of EB virus (EBV)-positive diffuse large B-cell lymphoma (DLBCL) with initial symptoms of retroperitoneal lymph node enlargement. Methods A retrospective analysis was conducted on the clinical data, imaging and pathological findings of a patient with EBV-positive DLBCL, and a literature review was performed. Results The patient was a 53-year-old female who underwent CT examination due to abnormal liver function and was found to have retroperitoneal lymph node enlargement. Gross pathology: two gray-white strip-like tissues, 0.4–0.6 cm in length, with a maximum diameter of 0.1 cm. Microscopic findings: under low power, the normal structure of the lymph node was completely destroyed, and cells with clear cytoplasm were found in the fibrous interstitium, showing diffuse proliferation of atypical large cells. Under high power, the cells showed significant anaplasia, scattered in a reactive background, with swollen nuclei, condensed nuclear chromatin, and frequent pathological mitotic figures. Immunohistochemical and in situ hybridization results confirmed the diagnosis of EBV-positive DLBCL. Conclusion EBV-positive DLBCL has an insidious clinical presentation and requires comprehensive diagnosis combining imaging, pathology, and immunophenotype.
