Cribriform morular thyroid cancer:report of a case and review of literature

Abstract:Objective: To explore the clinical and pathological characteristics of sclerotic-mulberry-like thyroid carcinoma (CMTC). Methods: A case of CMTC diagnosed by surgical specimens in Meizhou People's Hospital, Guangdong Province in June 2024 was reviewed, including clinical data, histological features, immunohistochemistry, and molecular testing. Combined with literature review, the clinical and pathological characteristics of CMTC were analyzed. Results: The patient was a female, 18 years old, with a mass in the anterior right neck and a foreign body sensation for one year. Grossly, the nodule had clear boundaries, gray-yellow cut surface, solid, and soft texture. Microscopically, the tumor infiltrated the capsule, showing glandular, sieve-like, and papillary growth patterns. The glandular epithelial cytoplasm was eosinophilic, with crowded nuclei, irregular nuclear membranes, faint chromatin, nuclear grooves, and nucleoli. Some cells showed transparent changes, and mulberry bodies were observed. Immunophenotype: The tumor showed diffuse strong expression of Galectin-3, CD56, β-catenin, and TTF1; glandular, sieve-like, and papillary structures showed strong positivity for CK19, ER, PR, and PAX8, while mulberry bodies showed moderate positivity for CDX2. Molecular testing: No mutations in KRAS, NRAS, or BRAF V600E genes were detected. Conclusion: CMTC is a rare type of thyroid cancer. Recognizing its unique morphological, immunohistochemical, and molecular features can help in clinical diagnosis and treatment, as well as screening for familial adenomatous polyposis (FAP).
