Colonic metastasis from esophageal squamous cell carcinoma complicated with colonic adenocarcinoma:a clinicopathological analysis and review of literature

Abstract:Objective To investigate the clinicopathological features of colonic metastasis from esophageal squamous cell carcinoma(ESCC)complicated with primary colonic adenocarcinoma.Methods A case of colonic metastasis from ESCC complicated with colonic adenocarcinoma,diagnosed in 2025 at Jinjiang Municipal Hospital(Shanghai Sixth People's Hospital Fujian Division),was collected.The morphological characteristics and immunohistochemical findings were analyzed,and the patient's treatment and prognosis were followed up.A literature review was also conducted.Results A 67-year-old male patient had undergone radical resection for mid-esophageal squamous cell carcinoma six months prior.He was admitted due to abdominal pain and diarrhea.Abdominal CT revealed space-occupying lesions in the transverse colon and sigmoid colon,which caused secondary intestinal obstruction.After colostomy and neoadjuvant therapy,radical subtotal colectomy was performed.Pathological examination showed squamous cell carcinoma infiltration from the submucosal to the extraserosal layer in the transverse colon,accompanied by intravascular tumor thrombus formation.In the sigmoid colon,moderately differentiated adenocarcinoma was identified,which invaded the submucosa.Immunohistochemistry revealed that the squamous cell carcinoma cells in the transverse colon were positive for CK7,CK5/6,34βE12,p40,p63,and EGFR,but negative for CK20,CDX2,SATB2,and p16.The Ki67 proliferation index was 50%-70%.The adenocarcinoma cells in the sigmoid colon were positive for CK20,CDX2,Villin,and SATB2,but negative for CK7,CK5/6,and p63.Conclusion The colon is a rare metastatic site for ESCC,and colonic metastatic ESCC must be differentiated from primary colonic squamous cell carcinoma.The complication of colonic adenocarcinoma is even rare.Diagnosis relies on clinical history,histopathology,and immunophenotype.Currently,there is no standardized treatment plan,palliative care is the main approach,and surgical treatment may be performed based on individual conditions.
