Invasive breast carcinoma with medullary features:a clinicopathological and molecular analysis of nine cases

Abstract:Objective To investigate the clinicopathologic characteristics,immunophenotype,differential diagnosis,and molecular features of breast carcinoma with medullary features(CMF).Methods Clinical and pathological data of 9 cases of breast CMF treated at Jiangyin Traditional Chinese Medicine Hospital from 2015 to 2022 were collected.Immunohistochemical staining,in situ hybridization(ISH)detection for Epstein-Barr virus(EBV),and next-generation sequencing(NGS)were performed,and relevant literature was reviewed.Results The patients were aged 37 to 75 years(median age:53 years).Morphologically,the tumors presented as syncytial cell clusters with a pushing growth pattern,indistinct cell borders,cellular atypia,high-grade nuclei,and prominent lymphocytic infiltration.Immunophenotypically,estrogen receptor(ER),progesterone receptor(PR),and human epidermal growth factor receptor 2(HER2)were negative.The Ki-67 proliferation index ranged from 60%to 90%.Basal cell markers were expressed,and p53 showed mutant-type expression.Molecular testing of 3 cases showed TP53 gene mutations,while ISH for EBER yielded negative results.Conclusion Breast CMF is a high-grade breast carcinoma of no special type with TP53 mutations.It belongs to the immunomodulatory subtype of triple-negative breast cancer,is not EBV-associated,and has a relatively favorable prognosis..
