Epididymal papillary cystadenoma with significant dilation of rete testis:report of a case

Abstract:Epididymal tumors are most commonly adenomatoid tumors and leiomyomas, while papillary cystadenomas are relatively rare. The coexistence of secondary significant expansion of the testicular network poses diagnostic challenges. This disease mainly needs to be differentiated from epididymal adenomatoid tumor, intermediate-grade serous tumor, and primary or metastatic epididymal adenocarcinoma. This article reports a case of papillary cystadenoma of the epididymis, and discusses its clinical and pathological features and histogenetic issues to enhance the understanding of such lesions among clinicians and pathologists. The histological structure of this case was mainly tubular, with rare papillary structures. Combined with immunohistochemical results, the tumor cells showed diffuse expression of CK-pan, CK7, PAX8, CD10, and AR, suggesting a origin from the Wolffian duct. At the same time, there was secondary significant expansion of the testicular network. These characteristics have not been reported in the literature.
