Abstract:Objective To investigate the clinicopathological features of extranodal NK/T-cell lymphoma (ENKTCL) with a rare phenotype. Methods A retrospective analysis was performed on the clinical and pathological characteristics, immunohistochemical findings, and gene rearrangement results of a case of ENKTCL accompanied by a connective tissue disease, along with a literature review. Results The lymph node biopsy showed scattered irregular atypical lymphoid infiltration with significant necrosis, locally distributed around blood vessels. Immunohistochemistry revealed that many lymphocytes were EBER positive. Double staining of CD3/EBER and CD79a/EBER indicated that the positive cells were neither B nor T cells. Further double staining of CD2/EBER showed that EBER-positive atypical lymphoid cells expressed only CD2 and not other T/NK cell markers. Both TCR and IG gene rearrangements were negative. Conclusion ENKTCL with abnormal CD2 expression is rare and can be misdiagnosed as other types of lymphoma. This case report demonstrates the diagnostic process and provides an in-depth analysis to help pathologists avoid diagnostic pitfalls.
