Predictive biomarkers of gastric-type endocervical adenocarcinoma

Abstract:Gastric-type adenocarcinoma (GEA) is the most common non-HPV-related cervical adenocarcinoma (ECAs), accounting for approximately 10% to 25% of cases. Compared with HPV-related ECAs, GEAs exhibit higher aggressiveness, extrauterine spread, and advanced staging. Due to the overreliance on HPV testing in cervical cancer screening, 40% to 100% of GEA patients are diagnosed at stage II to IV, with 75% presenting distant metastases such as ovarian, peritoneal, or omental involvement. Even patients with stage I disease show poor prognosis, often with distant metastasis and chemoresistance, resulting in a worse outcome compared to other types of ECAs. Clinical anti-HER2 therapy has been proven effective in various cancers, but it has not yet been applied to ECAs, including GEAs. The authors reviewed the literature on predictive biomarkers for ECAs, among which HER2 amplification, Claudin18.2, and PD-L1 protein expression show potential as predictive biomarkers for targeted and immunotherapy. However, existing data vary significantly, possibly due to differences in tumor type selection, detection methods, and scoring criteria. Therefore, there is an urgent need for clinical trials specifically designed for GEA patients to clarify the predictive value of HER2 amplification, PD-L1, and Claudin18.2 expression.
