Squamous cell carcinomas arising in intracranial epidermoid cysts:a clinicopathological analysis of one case

Abstract:Objective: To explore the clinical and pathological features, diagnosis, differential diagnosis, and treatment strategies of malignant transformation of intracranial epidermoid cysts (IECs) into squamous cell carcinomas (SCCs). Methods: A case of IECs transforming into SCCs was collected, and HE staining and immunohistochemical staining were performed to study its clinical and pathological characteristics, with a review of relevant literature. Results: The patient was a 54-year-old female who was admitted due to right eyelid ptosis for more than one month. Microscopic examination showed that the cyst wall was lined by stratified atypical hyperplastic squamous epithelium, with a large amount of keratinous material in the cyst cavity. Tumor cells infiltrated the cyst wall stroma, showing a sheet-like or nest-like distribution, with necrosis present. The cells were larger, with visible keratin pearls, mild to moderate nuclear atypia, and prominent nucleoli. Immunohistochemical staining showed positivity for p40, CK5/6, and p63, with a Ki-67 positive rate of approximately 30%. Conclusion: Malignant transformation of IECs into SCCs is very rare, and the diagnosis mainly relies on pathological examination. The current preferred treatment is surgical resection combined with radiotherapy.
