Abstract:Objective To investigate the clinical pathological features, immunophenotype, diagnosis, differential diagnosis, and treatment of renal-cell-like adenocarcinoma of the uterine endometrium. Methods Review of the literature and a retrospective analysis of the clinical pathological features, immunophenotype, differential diagnosis, and treatment of two cases of renal-cell-like adenocarcinoma of the uterine endometrium treated at The Second People's Hospital of Yibin City. Results Both cases were female patients, aged 63 and 71 years, respectively. Imaging suggested a uterine mass. Under the microscope, the tumor showed glandular, papillary, micropapillary, and tubular structures, with cells arranged in pseudostratified or single layers, oval or polygonal nuclei, and visible nucleoli. Immunohistochemistry showed PAX8 and CD10 positivity, TTF-1 and GATA-3 showed inverse expression, and there was no deficiency in mismatch repair proteins. One case did not receive postoperative treatment and died after a follow-up of 1.5 years; the other received paclitaxel plus carboplatin chemotherapy and remained stable after a 3-month follow-up. Conclusion Renal-cell-like adenocarcinoma of the uterine endometrium is a rare and highly malignant gynecological tumor, with a high age of onset, strong invasiveness, and tendency for vascular invasion. Diagnosis requires comprehensive analysis of histomorphology and immunophenotype to avoid misdiagnosis and missed diagnosis.
