A single-center study of cervical cancer screening in women aged 65 years and above in Xiamen

Abstract：Objective To investigate the characteristics of cervical cancer screening for women aged 65 years and above.Methods Collect cervical cancer screening data for women aged 65 and above at the Xiamen Maternity and Child Health Hospital between 2022 and 2023,including clinical symptoms,Liquid-based Cytology(LCT),Human Papillomavirus(HPV),and histopathological results.Using histopathological results as the gold standard,compare the screening efficacy of different screening methods.Results ① There were 680 study subjects,444 aged 65-69 years,164 aged 70-74 years,and 72 aged≥75 years;② The total HPV infection rate was 36.6％,and the total LCT positive rate was 19.1％.The rates in 65-69 years and 70-74 years were significantly higher than those in≥75 years(P＜0.01).The top four HPV types detected were 52,16,58,and 53;③ The Yoden index of different screening methods was compared,and the results showed that the HPV alone(0.584)was higher than the HPV combined with LCT(0.540)and the LCT alone(0.536);④ Follow-up showed that 62.7％of women had never received screening within 10 years.The detection rate of histopathological lesions among those who had never received screening within 5 years was significantly higher than that among those who had received screening 1 time or more within the same period(P＜0.01).Conclusion Attention should be paid to cervical cancer screening for women aged 65 years and above,especially those aged 65-69.They should continue to receive screening.The HPV type-specific detection method has good screening effects and can be combined with LCT results for comprehensive analysis.
