Pathological characteristics of labial glands in patients with primary Sjogren's syndrome and their clinical value

Abstract:Objective To investigate the pathological features of labial glands in patients with primary Sjogren's syndrome(pSS)and their clinical value.Methods Seventy-one patients with pSS were enrolled in the study.Based on the results of labial salivary gland biopsy(LSGB)pathology,the patients were divided into the positive group(LSGB≥grade 3)and the negative group(LSGB<grade 3).The differences in serum levels of anti-nuclear antibody(ANA),anti-SSA antibody,anti-SSB antibody,rheumatoid factor(RF),immunoglobulins,erythrocyte sedimentation rate(ESR),and C-reactive protein(CRP)between the two groups were compared.The relationship between LSGB and related indicators was analyzed.The positive group was further subdivided into mild and severe subgroups according to the degree of labial gland tissue damage.The differences between the two subgroups were compared,and the correlation between the degree of labial gland tissue damage and each clinical index were analyzed.Furthermore,the correlations between ANA titer,anti-SSB antibody titer,and each clinical index were analyzed.Results Among the 71 patients with pSS,52 were in the LSGB-positive group and 19 in the LSGB-negative group.Serum levels of IgA,RF and CRP in the LSGB-positive group were higher than those in the LSGB-negative group,and the differences were statistically significant(P<0.05).Binary logistic regression analysis showed that elevated ANA,IgM and CRP might have a certain impact on the pathological damage of the labial gland,and the difference was statistically significant(P<0.05).The SERUM levels of WBC,PLT,and IgG in the mild LSGB subgroup of pSS patients were lower than those in the severe LSGB subgroup,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that the disease duration,AST,IgG,and ANA were correlated with the degree of labial gland tissue damage.Conclusions Serum levels of RF,IgA,and CRP in pSS patients may be involved in the pathophysiological process of labial gland injury.The degree of labial gland tissue injury in pSS patients is correlated with the disease duration,as well as serum levels of ANA,IgA,IgM,IgG,globulin,RF,CRP,RBC,and PLT.
