Primary breast interdigitating dendritic cell sarcoma:report of a case

Abstract:Objective To explore the clinical pathological features, immunophenotype, diagnosis, and differential diagnostic points of dendritic cell sarcoma of the breast (IDCS). Methods A case of IDCS occurring in a female breast was reported. Results The patient was a 74-year-old woman who presented with a mass in the left breast 4 months ago. Magnetic resonance imaging showed a round or oval lesion with mixed signals on T1WI and T2WI in the upper outer quadrant of the left breast. Mammography revealed a high-density nodule above the left nipple with clear boundaries. Under light microscopy, there was extensive proliferation of epithelioid atypical cells within the breast tissue. These cells had single, double, or multiple nuclei, with prominent nucleoli, eosinophilic or foamy cytoplasm, and frequent mitotic figures. In the stroma, there was a large infiltration of acute and chronic inflammatory cells, mainly neutrophils and plasma cells. Immunohistochemical results showed diffuse positivity for S-100, and focal positivity for CD68 and CD163. Conclusion This is the first reported case of dendritic cell sarcoma of the breast (IDCS) in China. Due to the rarity of this disease and its occurrence in a special location such as the breast, pathologists and clinicians should enhance their awareness of this condition to reduce the risk of misdiagnosis and missed diagnosis.
