Gastric hamartomatous inverted polyp:report of a case and review of literature

Abstract:Objective To explore the clinical and pathological features of gastric hamartomatous inverted polyp (GHIP). Methods A case of GHIP was analyzed retrospectively in terms of its clinical and pathological characteristics. Results The patient was a 48-year-old male who was admitted to the hospital due to abdominal distension for more than half a month. Endoscopic ultrasonography showed a raised lesion on the greater curvature of the upper body of the stomach, with central depression, mucosal erosion, and a mixed low echo lesion in the gastric proper muscle layer. Endoscopy: submucosal cystic dilation of glands showed a lobulated distribution, with proliferation of surrounding branch smooth muscle bundles and visible smooth muscle boundaries. Immunohistochemistry: MUC5AC (+ in dilated glands), MUC6 (+ around dilated glands), CK7 (+ in part), CK20 (+ mainly in dilated glands), P53 (weak + in small amount), Ki-67 (+ mainly in gland neck), CDX2 (-), Desmin (+ in proliferating smooth muscle), Syn (+ in scattered neuroendocrine cells). The pathological diagnosis was GHIP. Conclusion GHIP is rare, and a definitive diagnosis requires histological features, immunophenotype, as well as endoscopic and ultrasonic findings.
