Clinicopathological and molecular features of breast mucinous carcinomat:report of ten cases

Abstract:Objective To investigate the clinicopathological features,immunophenotype,differential diagnosis,molecular features of breast mucinous carcinoma.Methods A total of 10 cases of breast mucinous carcinoma were collected from our pathology department between 2015 and 2024.These cases were analyzed using immunohistochemical staining,fluorescence in-situ hybridization and next-generation sequencing,as well as reviewing related literatures.Results The 10 cases were female with age range of 43-84 years(mean 61.5 years).Histologically,the cases exhibited distinct features such as clusters,glandular,trabecular,cribriform,papillary,micropapillary,and nest-like structures in extracellular mucus,with mucus component accounting for 60%-90%(more than 90%)of the total.Immunophenotypically,ER was positive(10/10),PR positive(7/10),HER2 negative(9/10),and Ki-67 index ≤ 20%(8/10);Molecular features included GATA3 shifted code mutation,ERBB2 and MDM2 copy number variation,MET missense mutation,but no common mutations of breast carcinoma(PIK3CA,TP53,BRAC1/2)were identified.Conclusion Breast mucinous carcinoma lacks the high-frequency PIK3CA mutations typically associated with hormone receptor-dependent breast carcinoma,indicating potential clinical significance of other mutations such as GATA3 and MET etc.More cases may warrant further investigation into these molecular features.
