Bronchiolar adenoma with micropapillary features:multidimensional clinicopathological analyses

Abstract:Objective To investigate the clinicopathological characteristics of bronchiolar adenoma(BA)with micropapillary features.Methods A retrospective analysis was conducted on four BA cases with micropapillary patterns pathologically confirmed at the First Affiliated Hospital of Xi'an Jiaotong University from January 2019 to January 2024.Clinical symptoms,radiological characteristics,and histopathological data were collected.Next-generation sequencing(NGS)was utilized to detect genetic alterations of whole genome.Results The BA patients with micropapillary features aged from 39 to 69 years,with a maximum lesion diameter of 13 mm.Histologically,micropapillary cell clusters floating within pulmonary alveolar spaces were observed.One case exhibited the typical bi-layered structure,while three cases showed partial loss of the basal cell layer,accompanied by abundant ciliated or mucinous cells.Genetic alterations of HRAS,SETD2,EMSY,and RNF43 were detected in two BA cases.No adjuvant therapy was administered postoperatively,and no recurrence or metastasis was observed during follow-up(12-67 months).Conclusion Multidimensional and comprehensive clinicopathological analysis is critical for BA diagnosing with micropapillary features,which may enhance the recognition of this distinct pulmonary tumor entity among pathologists and clinicians.
