Clinicopathological features of rare primary carcinosarcoma of lung

Abstract:Objective To investigate the clinicopathological features,diagnosis,differential diagnosis,treatment and prognosis of primary lung carcinosarcoma.Methods The clinical data,histopathological and immunohistochemical features,treatment and prognosis of 2 cases of rare primary pulmonary carcinosarcoma diagnosed in the Department of Pathology,the First People's Hospital of Guangyuan were retrospectively analyzed.Results There were 1 case of elderly female and 1 case of male.Microscopically,the tumor contained two components:cancer and sarcoma.One case of cancerous component was moderately differentiated squamous cell carcinoma,and immunohistochemistry showed positive expression of CK5/6,p63 and p40.The other case of cancerous component was acinar adenocarcinoma,and immunohistochemistry showed positive expression of CK7,TTF-1 and Napsin-A.Immunohistochemistry of sarcoma compament showed positive expression of Vimentin and CD31.Conclusion Primary carcinosarcoma of the lung is a highly malignant tumor with epithelial and mesenchymal biphasic differentiation.It is rare,aggressive,poor prognosis,lack of specific clinical symptoms and imaging findings.Histopathology and immunohistochemistry are helpful for diagnosis and differential diagnosis to prevent misdiagnosis and missed diagnosis.
