Clinicopathological features of intrathyroidal thymic adenocarcinoma:report of three cases and review of the literature

Abstract:Objective To investigate the clinicopathological features of intrathyroidal thymic carcinoma(ITTC)and to enhance clinicians' understanding of this disease.Methods We retrospectively analysed the clinicopathological features of three cases of ITTC,applied HE,immunohistochemistry and genetic testing to detect the expression of p63,CD5,CD 117 and so on in ITTC,and searched and reviewed the relevant domestic and international literature.Results The three cases of ITTC were all poorly defined solid nodules on the right side of the thyroid gland,with an average maximum diameter of about 3.3 cm.Microscopically,atypical epithelial cells with vesicular nuclei were arranged in irregular sheets or nests,and obvious fibrotic intervals were seen,accompanied by varying numbers of lymphocytic infiltration,and the tumour cells all expressed p63,CD5,CD 117,p40,and CK19,and none of them expressed TTF-1;One case was wild-type by BRAF V600E second-generation sequencing,and one case was wild-type by 10-gene test for thyroid cancer.Conclusion ITTC is a rare malignant tumour in the thyroid gland,which lacks specific molecular features and is more prone to recurrence and metastasis than papillary thyroid carcinoma,and immunohistochemistry CD5 and CD117 are reliable indicators for the diagnosis of ITTC.
