Pathological diagnosis and literature review of lipofibroadenoma of the thymus

Abstract:Objective To explore the clinical pathological features, diagnosis, and differential diagnosis of thymic lipofibroadenoma (LFA). Methods A case of LFA was reported, combined with 27 cases from the literature, a retrospective analysis of the clinical pathological characteristics of this tumor was conducted to enhance understanding of this tumor. Results The tumor occurred in a wide age range, without specific clinical symptoms, with a diameter of 3-26 cm, encapsulated, and the cut surface was gray-white or gray-yellow, soft or tough in texture. Under the microscope, the tumor consisted of mature adipose tissue and proliferative fibrous tissue in different proportions, with epithelial components interspersed in the fibrous tissue. The number of epithelial cells varied, distributed as slit-like or nest-like within the fibro-fatty tissue, morphologically similar to breast fibroadenoma, and residual thymic tissue was seen in surrounding tissues. Immunohistochemical markers showed that epithelium expressed AE1/AE3, CK19, p63, CK5/6, could express EMA, CK7, CK8/18, p40, lymphoid tissue around the epithelium expressed CD3, CD20, CD5, medullary islands expressed TDT, and the epithelial component showed low expression of Ki-67. In all reported cases, follow-up ranged from six months to ten years, with no recurrence cases. Conclusion Thymic lipofibroadenoma is a rare benign thymic tumor, its pathogenesis is still unclear, diagnosis relies on pathological diagnosis, and treatment is mainly surgical resection.
