Clinicopathological features of autoimmune gastritis with gastric epithelial neoplasms

Abstract:Objective To explore the tumor lesions related to autoimmune gastritis(AIG),especially pathological types and histomorphological characteristics of gastric epithelial tumours,in order to increase the awareness of AIG and AIG-related diseases.Methods Cases diagnosed as gastric epithelial tumours with AIG were collected from the Pathology Department of Liaocheng People's Hospital from January 2019 to February 2023.Their clinical and pathological data were analyzed and the literature review was conducted.Results In all,5 confirmed and 7 probable AIG cases were identified.There were 5 females and 7 males,and the average age was 61 years(range 44-76 years).There were 4 cases with anemia and 2 cases with diabetes.2 patients presented with fatigue,while the remaining 10 had digestive symptoms.7 patients had neuroendocrine cell hyperplasia and 5 had neuroendocrine neoplasm in the proximal stomach.The types of epithelial gastric neoplasms include:3 cases of pyloric gland adenoma,1 case of malignant transformation of hyperplastic polyps with infiltration into the submucosa,6 cases of high-grade intraepithelial neoplasia(1 case with metachronous multiple neoplasms),1 case of poorly differentiated adenocarcinomas with signet ring cell cancer diagnosed by biopsy,1 case of metachronous multiple high-grade intraepithelial neoplasia,intramucosal and submucosa-invasive adenocarcinoma.According to the expression of mucin in tumor cells in gastric cancer,the mucin classification of gastric epithelial tumors showed that 7 cases were gastric type,2 cases were intestinal type,1 case with no mucin expression,and 2 cases were unknown.Conclusion AIG may lead to pernicious anemia(PA)and is often observed together with other autoimmune diseases.It is associated with neuroendocrine neoplasm,hyperplastic polyps and pyloric gland adenoma,among which gastric hyperplastic polyps may go through carcinomatous conversion.The mucinous classification of the concomitant gastric epithelial tumors is usually of the gastric type.When AIG is diagnosed,long-term follow-up and symptomatic treatment are required.Identifying patient characteristics that are associated with AIG is crucial for clinicians and pathologists.
