Triple-negative invasive micropapillary carcinoma of the breast with colonic metastasis:report of a case and review of literature

Abstract:Objective To explore the clinical and pathological features and diagnostic points of triple-negative invasive micropapillary carcinoma of the breast with colonic metastasis. Methods The diagnosis and differential diagnosis of the patient's breast local excision and colonoscopy biopsy lesions were performed by routine HE staining and immunohistochemical staining. The clinical pathology characteristics, immunophenotype, and treatment were analyzed, and related literature was reviewed. Results Tumor cell nests with micropapillary carcinoma characteristics were found in both breast and colon biopsy tissue sections. Under low magnification, the tumor cells showed a micropapillary pattern, with gaps between the tumor cell nests and the stroma; under high magnification, the cytoplasm was eosinophilic, and the nuclei showed intermediate nuclear morphological changes. The immunophenotypes were similar: ER, PR, HER-2, GCDFP-15, and GATA-3 were negative, EMA was positive at the periphery of the micropapillary structures, and TRSP1 and CK7 were positive. Conclusion Clinical and colonoscopic manifestations of colonic metastatic invasive micropapillary carcinoma of the breast lack specificity, and the metastasis of triple-negative breast cancer increases the difficulty of pathological diagnosis. Therefore, the diagnosis of metastatic triple-negative breast cancer requires comprehensive consideration of histological morphology, immunophenotype, and clinical history.
