Abstract:To explore the clinical and pathological characteristics of malignant pleural effusion caused by scrotal Paget's disease. This study analyzed the clinical and pathological data of one case of pleural effusion metastatic scrotal Paget's disease, observing the cellular morphology and immunohistochemical features. The patient was sent for cytological examination of pleural fluid due to bilateral pleural effusion. Under the microscope, tumor cells were scattered or arranged in nests on the smear, with large cell bodies, large nuclei, granular chromatin, prominent nucleoli, and abundant, dense cytoplasm, mostly eosinophilic; in the cell block, some tumor cells showed pale or clear cytoplasm. Immunohistochemistry showed that the tumor cells were positive for GCDFP-15, AR, CKpan, CK7, CK8/18, and negative for TTF-1, P40, CK20, Villin, CD56, and Calretinin. Scrotal Paget's disease with pleural effusion metastasis is extremely rare. Based on cytological features, combined with medical history and immunohistochemistry, a definitive diagnosis can be made.
