Abstract:Cervical squamous cell carcinoma is the most common type of cervical cancer, often originating from precancerous lesions—high-grade squamous intraepithelial lesions (HSIL). Clinical cases of HSIL spreading to involve the endometrium are rare, and preoperative examinations are difficult to detect. This article reports a case of a 69-year-old elderly female patient with diffuse involvement of the endometrium by cervical HSIL. A retrospective analysis was conducted on her clinical manifestations, imaging findings, postoperative histopathological morphology, and immunophenotypic characteristics. Combined with relevant literature, the pathogenesis, pathological diagnosis, differential diagnosis, clinical treatment, and prognosis were discussed. Histological examination showed that the cervical HSIL diffusely involved the surface of the endometrium without residual normal endometrial glandular epithelium. Immunohistochemistry revealed that the endometrial HSIL was strongly positive for p16, p40, CK5/6, and p63. The patient underwent total hysterectomy with bilateral salpingo-oophorectomy and pelvic lymph node dissection, followed by adjuvant radiotherapy and chemotherapy. After 11 months of postoperative follow-up, no tumor recurrence or metastasis was observed.
