Stratified mucin-producing neoplastic lesions of the cervix:a clinicopathological analysis

Abstract:Objective: To investigate the clinical and pathological characteristics, diagnosis, and differential diagnosis of mucin-producing squamous intraepithelial lesions (SMILE) and invasive mucin-producing squamous carcinoma (ISMC). Methods: Six cases of mucin-producing squamous epithelial neoplastic lesions of the cervix were collected, and their clinical and pathological features, immunophenotype, and special staining characteristics were analyzed. Relevant literature was reviewed. Results: Two cases were SMILE, and four cases were ISMC, with ages ranging from 28 to 44 years. All cases were positive for high-risk HPV. One case was SMILE combined with HSIL; one case was a conventional invasive adenocarcinoma combined with cervical adenocarcinoma in situ (AIS), including SMILE and conventional AIS; one case was ISMC combined with adenosquamous carcinoma and HSIL; one case was invasive adenocarcinoma, including ISMC, conventional, and intestinal-type invasive adenocarcinoma; one case was ISMC combined with HSIL; one case was ISMC combined with conventional AIS. Immunophenotype: p16, CK7, CEA showed diffuse or partial positivity, Ki-67 proliferation index ≥80%, while ER, PR, p63, p40 were negative. AB-PAS and mucicarmine staining showed varying amounts of mucin within tumor cells. Follow-up ranged from 1 to 56 months, with no recurrence or metastasis. Conclusion: Mucin-producing squamous epithelial neoplastic lesions of the cervix are a rare lesion associated with high-risk HPV infection, characterized by unique histological features and immunophenotype. Accurate diagnosis of these lesions is of great significance for guiding clinical treatment.
