Abstract:Objective To investigate the histomorphological features, diagnosis, differential diagnosis, treatment, and prognosis of cutaneous involvement in multisystem EBV-positive diffuse large B-cell lymphoma (DLBCL). Methods A case of cutaneous EBV-positive DLBCL was analyzed using HE staining, immunohistochemistry, EBER in situ hybridization, and Ig/TCR gene rearrangement testing. Results The patient was a male, 83 years old, with a gradually enlarging mass on the left upper limb over 3 months. Histologically, the lesion showed moderately to highly atypical medium-to-large cells scattered singly or in clusters within an epithelioid granulomatous and reactive inflammatory background. 80% of the atypical cells were EBER positive, and IgH and κ light chains were detected. Conclusion Cases of multisystem EBV-positive DLBCL with significant granulomatous reaction in the skin are very rare. It is necessary to complete the medical history, imaging studies, and related laboratory tests to clarify the involvement of lymph nodes and other extranodal systems, and to differentiate from other types of lymphomas to avoid misdiagnosis and missed diagnosis.
