Clinicopathological features of malignant epithelial tumor with differentiation of yolk sac tumor:report of six cases and review of literature

Abstract:Objective To investigate the clinicopathological features and prognosis of malignant epithelial tumor with differentiation of yolk sac tumor.Methods The clinicopathological data of 6 cases of malignant epithelial tumor with differentiation of yolk sac tumor were selected from 2022 to 2023 in the First Affiliated Hospital,Sun Yat-sen University.Results The patients ranged in age from 41 to 68 years,with average of 55 years.There were 3 cases of abnormal uterine bleeding and 3 cases of abdominal distension.5 cases occurred in ovary,and 1 case in corpus uteri.Histopathological types included high-grade serous carcinoma(4 cases),clear cell carcinoma(1 case)and carcinosarcoma(1 case).The FIGO stage of 5 tumors showed stage I in 2 cases,stage II in 1 case,and stage III in 2 cases.5 patients underwent panhysterectomy and bilateral salpingo-oophorectomy,and 3 of 5 underwent pelvic lymphadenectomy and paraortic lymphadenectomy.A preoperative serum AFP was elevated in 4 cases.4 cases were in treatment and 2(33%)cases died during follow-up.Yolk sac tumor areas were consistently positive for SALL4(6/6)and Glypican3(5/5).Meanwhile,AFP was positive in 5 cases(5/6),P53 mutant expression in 5 cases(5/6),and CK7 positive in 3 cases(3/5).Conclusion The malignant epithelial tumor with differentiation of yolk sac tumor has high malignant characteristics and rapid development and high mortality rate.Serological AFP detection and immunohistochemistry of SALL4,Glypican-3 and AFP are important for diagnosis.
