Characteristics of bone marrow biopsy in childhood myelodysplastic neoplasms:a clinicopathological analysis of eight cases

Abstract:Objective To explore the clinicopathological characteristics,differential diagnosis,and prognosis in pediatric patients with myelodysplastic neoplasms(MDN)that underwent bone marrow biopsy.Methods Clinical and pathological data from eight pediatric patients diagnosed with MDN were collected and summarized.Results Among the patients,six were male and two were female,with one or more peripheral blood lineages decreased.Bone marrow biopsy showed extremely hypoplastic marrow in three cases,hypoplastic marrow in one case,hyperplastic marrow in one case,and extremely hyperplastic marrow in three cases.Abnormal localization of blast cells was observed in seven cases,and varying numbers of primitive cells and dysplastic megakaryocytes were present in all eight cases.The number of CD34-positive primitive cells in bone marrow biopsy was>5%in four cases and≤5%in the remaining cases.CD42b-labeled dysplastic megakaryocytes accounted for 5%-20%of all megakaryocytes.Fibrosis was graded as 0 in two cases,1 in four cases,and 2 in two cases.Cytodysplasia was observed in bone marrow smears of six cases,an increased proportion of flow cytometry-detected primitive cells in four cases,abnormal karyotypes in four cases,CEP7D7S486(7q31)gene deletion in one case,and MDS-related gene mutations in four cases.Three cases were lost to follow-up,two died,and three survived.Conclusion Pediatric myelodysplastic neoplasms are relatively rare,and bone marrow biopsy combined with other laboratory tests has an important diagnostic value.
