Expression of MHC-I and MHC-II in diffuse large B-cell lymphoma and their relationship with clinical pathological features and prognostic impact

Abstract:Objective To investigate the expression of major histocompatibility complex(MHC)I and II in diffuse large B-cell lymphoma(DLBCL)and their relationship with clinicopathological features and prognosis.Methods A total of 130 cases of DLBCL in the Department of Pathology,the First Affiliated Hospital of Xinjiang Medical University were collected,and the expression of MHC-Ⅰ and MHC-Ⅱ proteins was detected by immunohistochemical EnVision method,and the correlation between the two and the clinicopathological characteristics and prognosis of DLBCL was analyzed.Results The immunohistochemical staining Results demonstrated that the low expression rates of MHC-Ⅰ and MHC-Ⅱ in DLBCL tumor cells were 57.6%(68 out of 118 cases)and 40.3%(48 out of 119 cases),respectively.The low expression of MHC-Ⅰ was correlated with Ann-Arbor staging,performance status(PS)score and the location of occurrence(P<0.05),that of MHC-Ⅱ was associated with age and extranodal lesion(P<0.05),and the co-high expression of MHC-Ⅰ and MHC-Ⅱ was related to the sparing of bone marrow(P<0.05).Kaplan-Meier univariate survival Results showed poor prognosis in the group with low expression of MHC-Ⅱ(P<0.05),and MHC-Ⅰ had no significant effect on survival(P>0.05).Multivariate COX regression analysis showed that LDH,PS score and IPI score were independent risk factors affecting DLBCL patients.Conclusion The low expression of MHC-Ⅰ and MHC-Ⅱ in DLBCL is related to Ann-Arbor stage,PS score,site of occurrence,age,and extranodal lesions.Patients with low MHC-Ⅱ expression of DLBCL have a poor prognosis.
