Abstract:Objective To discuss the clinical manifestations, pathological diagnosis, and differential diagnosis of ectopic splenic implantation in the thoracic cavity. Methods A case of a patient with splenic implantation was collected, and related literature was reviewed. Results The patient was a 20-year-old male who had been found to have multiple pleural nodules for more than 5 months during a physical examination. Surgery was performed to remove the lesions. Microscopically, multiple nodules of varying sizes were observed, encapsulated by fibrous tissue, containing multiple lymphoid follicles, rich sinusoids lined by obvious endothelial cells filled with blood. Immunohistochemical results showed positivity for FⅧ, CD34, CD3, CD20, CD21, and CD235α, with a Ki-67 proliferation index of approximately 5%. Bcl6, CD10, CD61, CD68, and Cyclin D1 were all negative. Conclusion Ectopic splenic implantation in the thoracic cavity is usually asymptomatic and often discovered incidentally, which can easily be misdiagnosed as other benign or malignant thoracic lesions, leading to significant differences in treatment and prognosis. Therefore, accurate preoperative examination and diagnosis can help avoid unnecessary treatments to some extent.
