Clinicopathologic analysis of female adnexal tumor of probable Wolffian origin

Abstract:Objective To investigate the clinical and pathological characteristics and diagnostic points of female adnexal tumors possibly originating from the Wolffian duct. Methods Clinical data of 2 patients with female adnexal tumors possibly originating from the Wolffian duct were collected from Jiangsu North People's Hospital, combined with relevant literature to analyze the clinical pathological features, immunohistochemical phenotype, and differential diagnosis of this tumor. Results The two patients were aged 25 and 42 years, respectively, with lesions located in the right broad ligament and left broad ligament, respectively. Both masses had clear boundaries. Microscopically, proliferating epithelioid cells were seen, distributed in a sheet-like or cord-like pattern, and lined within slit-like and glandular structures. Immunohistochemistry showed that tumor cells were positive for CK, Vimentin, calretinin, and α-inhibin, and negative for EMA and CD99. Conclusion Tumors possibly originating from the Wolffian duct in female adnexa are rare. These tumors are usually located in the broad ligament or the mesosalpinx, which are sites of Wolffian duct remnants. Diagnosis should be based on histological features and location, and should be differentiated from sex cord-stromal tumors, adenomatoid tumors, and granulosa cell tumors.
