Abstract:Objective To study the clinical and pathological features, immunohistochemical characteristics, diagnosis and differential diagnosis, treatment, and prognosis of invasive urothelial carcinoma with chordoma-like features. Methods A case of invasive urothelial carcinoma with chordoma-like features occurring in the ureter was retrospectively analyzed. Clinical data were collected, and HE staining, immunohistochemistry, and special staining were performed to analyze the histopathological features, and relevant literature was reviewed. Results A male elderly patient showed a soft tissue mass shadow in the middle segment of the left ureter on CT urography (CTU). Histologically, the tumor had a basophilic mucoid stroma, with atypical urothelial cells arranged in cords or microcystic patterns. Immunohistochemistry showed CK-H (+), GATA3 (+), p63 (+), and PAS staining revealed extracellular mucin positivity, but no intracytoplasmic mucin. Pathological diagnosis: Invasive urothelial carcinoma with chordoma-like features. Conclusion Invasive urothelial carcinoma with chordoma-like features is a rare special type of urothelial carcinoma, characterized by chordoma-like morphology, but not by chordoma-like differentiation or immunophenotype. The definitive diagnosis relies on histopathology, immunohistochemistry, and special staining.
