Cytomorphological characteristics of intrathyroid thymic carcinoma with fine-needle aspiration

Abstract:Objective To investigate the fine needle aspiration cytology(FNAC)characteristics of intrathyroid thymic carcinoma(ITTC).Methods The clinical data,FNAC features,histological features,immunophenotype and BRAF V600E gene detection results of a patient diagnosed with ITTC were retrospectively analyzed,and relevant literature was reviewed.Results The patient was a 70-year-old woman;the tumor was located in the lower part of the left thyroid lobe,with a maximum diameter of 15 mm.Features of FNAC:The tumor appeared as large cell clusters and individual cells;the nuclei of the tumor cells were crowded,round,finely granular,moderately hyperchromatic,mild-moderately enlarged and atypia,and slightly thickened nuclear membrane;small nucleoli could be seen;the tumor cytoplasm was scarce with indistinct border and high nuclear-to-cytoplasmic ratio.The cytological diagnosis was epithelial tumor,suspected to be malignant.Postoperative tumor tissue positively expressed CD5,CD117,p40,Cyclin D1,CK5/6,Calectin-3,and p53 mutants,and a small amount of tumor cells expressed CgA,Syn,and HBME-1.Histological diagnosis was ITTC.BRAF V600E gene detection was wild type.Conclusion ITTC is a rare tumor.It is important for differentiating ITTC between similar tumors with poor prognosis by cytology to know the FNAC characteristics of ITTC,and avoiding excessive preoperative treatment.
