Clinical and pathological analysis of NUT carcinoma in nasal cavity and paranasal sinuses

Abstract:Objective To study the clinical characteristics,pathological diagnosis,differential diagnosis,treatment,and prognosis of nuclear protein in testing carcinoma(NC)in the nasal cavity and paranasal sinuse.Methods The clinical data,histological morphology and immunohistochemical features were analyzed in 2 cases of NUT carcinoma in the nasal cavity and paranasal sinuse,and relevant literature was reviewed.Results Under low magnification,the tumor boundary was unclear,and it grew infiltratively in the fibrous stroma in a cord like,nest like,or patchy manner.The tumor was composed of small to medium-sized poorly differentiated or undifferentiated cells,with obvious atypia,visible mitotic figures and apoptotic bodies,accompanied by patchy necrosis,and a large number of neutrophils infiltrating the stroma.In case 1,tumor cells were round or oval.They were relatively uniform in morphology without significant pleomorphism,with high cellular density,hyperchromatic nuclei,and a high nuclear-to-cytoplasmic ratio,distinct nucleoli.In case 2,tumor cells appear epithelioid in appearance,with a small to moderate amount of cytoplasm that was pale to eosinophilic red in color.Some cytoplasm was transparent,and the nucleus had vacuolated chromatin with obvious nucleoli.Most cells were immature in differentiation,and a few cells appeared to have squamous differentiation.Immunohistochemistry revealed that the tumor cells were diffusely positive for CK,INI1,SMARCA4and NUT,partial positive for p40 and CAM5.2,and negative for Syn,CgA,CD56,S100,SOX10,TTF1,INSM1,desmin,and SMA.The Ki-67 proliferation index ranged from 50%to 80%,and EBER in situ hybridization was negative in 2 cases.NUT gene rearrangement was detected in 2 cases by FISH.Conclusion NUT carcinoma of the nasal cavity and paranasal sinuse is rare,and its clinical features and imaging examinations have no obvious specificity.Histological examination often shows poorly differentiated or undifferentiated morphology,which is prone to misdiagnosis and missed diagnosis.Accurate diagnosis of NUT carcinoma requires integration of immunohistochemistry and molecular testing.
