The value of immunohistochemical CD34 and CD117 in the diagnostic stage of chronic myeloid leukemia

Abstract:Objective To explore the value of CD34 and CD117 immunohistochemical staining in the diagnosis and staging of chronic myeloid leukemia(CML).Methods A total of 103 cases of chronic myeloid leukemia(CML)from January 2022 to December 2023 were collected from the Pathology Department of Beijing HST Medical Laboratory.Retrospective analysis was conducted using methods such as bone marrow biopsy,immunohistochemistry,and Gomori staining,with a focus on observing the differential expression of CD34 and CD117 in the CML chronic phase(CP)and CML blast phase(BP)stages by immunohistochemistry.CD34 expressing primitive cells and CD117 expressing granulocyte precursor cells(primitive granulocytes and early promyelocytes)are classified into three patterns based on the morphological characteristics and percentage values of positive cell expression:Type I(scattered or clustered positive cells,<5%),Type II(mottled positive cells,6%-20%),and Type III(diffuse positive cells,>20%).Results CD34 and CD117 were mainly expressed in type I during the chronic phase(CP)of CML,with type I expression being greater than type II expression.In the blast phase(BP)of CML,type III expression was predominant,with type III expression being greater than type II expression.Conclusion Through bone marrow HE staining and Gomori staining,combined with the expression characteristics of CD34 and CD117 in immunohistochemistry,it can well meet the needs of the diagnosis and staging of CP and BP in CML bone marrow biopsy pathology.
