Pathologicalfeatures and immunophenotypic analysis of terminal deoxynucleotidyl transferase(TdT)-negative lymphoblastic lymphoma/acute lymphocytic leukemia

Abstract：Objective The purpose of this article is to explore the expression of Terminal Deoxynucleotidyl Transferase(TdT)in lymphoblastic lymphoma/acute lymphocytic leukemia(LBL/ALL)and to analyze its pathological characteristics and immunophenotypic presentation.Methods Collected 32 cases each of TdT-negative and TdT-positive lymphoblastic lymphoma/leukemia diagnosed at the First Affiliated Hospital of Xinjiang Medical University from 2010 to 2024,observed their histological morphology,pathological characteristics,and immunological manifestations,and reviewed them in conjunction with relevant literature from both domestic and international sources.Results TdT-negative ALL/LBL patients consisted of 21 cases of T-LBL/ALL and 11 cases of B-LBL/ALL,with 26 males and 6 females;while TdT-positive ALL/LBL patients included 13 cases of T-LBL/ALL and 19 cases of B-LBL/ALL,with 19 males and 13 females.Conclusion TdT-negative LBL/ALL is a rare and highly aggressive hematological malignancy.Since TdT is an important biomarker for the diagnosis of LBL/ALL,TdT negativity poses certain diagnostic challenges.A comprehensive analysis that combines clinical features,pathological characteristics,and immunophenotyping is necessary to establish a clear diagnosis and avoid misdiagnosis.
