Sinonasal primary extramedullary solitary plasmacytoma with Epstein-Barr virus infection:report of a case

Abstract:This article reports a rare case of extramedullary solitary plasmacytoma of the nasal cavity with EBV infection. The patient is a 30-year-old female who presented with "recurrent nasal obstruction and purulent rhinorrhea for half a year, with worsening one week." She was admitted to the hospital with a diagnosis of "nasal neoplasm" in the outpatient department. On admission, a new growth was found in the right nasal cavity and inferior meatus, measuring approximately 0.5 cm × 0.5 cm. CT diagnosis: space-occupying lesion in both nasal cavities, with a high possibility of polyp. Clinical surgery was performed, and the postoperative pathological diagnosis was extramedullary solitary plasmacytoma of the nasal cavity with EBV infection. Plasmacytomas are commonly seen in elderly males, and cases of EBV-infected plasmacytomas are mostly individual reports. Cases occurring in young females are rare, and those with lymph node metastasis are even rarer. EBV-positive plasmacytoma is a biological subtype of plasmacytoma, which may lead to diagnostic uncertainty. This case is reported along with a review of relevant literature to enhance the clinical and pathological understanding of plasmacytoma with EBV infection in the nasal and paranasal sinuses among clinicians and pathologists, thereby improving diagnosis and guiding treatment.
