Dieulafoy disease of gastric body:report of a case

Abstract:Dieulafoy disease of the stomach, also known as the constant-caliber artery syndrome, is a congenital vascular malformation in which the gastric feeding arteries maintain their diameter as they pass through the muscularis propria to the submucosa. Dieulafoy disease presents with hidden bleeding sites, large blood loss, and high mortality rates, making it an important yet rare cause of upper gastrointestinal bleeding. If clinical intervention is not timely, patients may develop hemorrhagic shock and face life-threatening conditions. Endoscopic treatment is the preferred diagnostic and therapeutic approach for Dieulafoy disease in the digestive tract, with a hemostasis success rate exceeding 90%. Only a few cases that fail endoscopic treatment proceed to surgical intervention. This article reports the diagnostic and therapeutic process and pathological changes of a patient with Dieulafoy disease in the body of the stomach. The patient experienced multiple episodes of active bleeding over six years, with no clinical manifestations during the remission period, and ultimately underwent total gastrectomy. Through case analysis and a literature review, this article discusses the pathological changes and clinical treatment of Dieulafoy disease, aiming to further enhance the understanding of this condition among clinicians and pathologists, so that more accurate and efficient diagnosis and treatment can be achieved in clinical practice.
