Solid-type breast adenoid cystic carcinoma with negative MYB-NFIB fusion gene:report of a case and review of literature

Abstract:Objective To explore the clinicopathologic features,diagnosis,differential diagnosis and molecular genetic features of Breast adenoid cystic carcinoma(BACC).Methods The clinicopathologic data of a patient with BACC were collected and relevant literature was reviewed.Results The patient was a female,78 years old,with a right breast mass with pain.Microscopically:the tumor was solid nested and map-like,and the solid area was>90%,the interstitium was collagenized with mucoid and vitreous degeneration,the tumor cells were moderately-heavily heterogeneous,and nuclear schizophrenia was common;immunophenotypes:CD117,CK5/6,p63,SMA,S-100,SOX-10,E-cadherin,and p120 were all positive,and p53 was wild-type,and the hotspot area had a Ki-67 proliferation index of about 50%,ER,PR,HER-2,AR,GATA-3,CD56,CgA,Syn were negative.MYB gene breaks were detected,but MYB-NFIB gene fusion was negative.Conclusion BACC are mostly low-grade malignant tumors with MYB-NFIB gene fusion,but the molecular features and driving mechanisms of MYB-NFIB fusion-negative cases need further in-depth study.
