Molecular classification and clinical features of posterior fossus ependymoma:report of 56 cases

Abstract:Objective To investigate the molecular types,clinicopathological features and prognosis of posterior fossa ependymoma.Methods A total of 56 cases of posterior craniocerebral ependymoma surgically removed in the First Affiliated Hospital of Xinjiang Medical University from January 2015 to June 2023 were collected,and their clinicopathological features were analyzed.The expression of H3K27me3 was detected by immunohistochemical method,molecular typing was performed,and prognostic correlation was analyzed.Results Among the 56 patients,28 were male and 28 were female.There were 30 cases of WHO grade 2 and 26 cases of WHO grade 3.The follow-up period for the 56 patients ranged from 5 to 105 months,with an average follow-up period of 43.53 months.24 patients were lost to follow-up,among whom 14 relapsed and 6 died.The median survival time was 39 months.There were 36 cases with H3K27me3 deficiency(PFA group)and 20 cases without H3K27me3 deficiency(PFB group).The proportion of patients under 18 years old in the PFA group was significantly higher than that in the PFB group(P<0.001),and the proportion of WHO grade 3 in the PFA group was significantly higher than that in the PFB group(P<0.05).The prognosis of the PFA group was worse than that of the PFB group,and there was a statistically significant difference in overall survival between the two groups(P=0.014,<0.05).Age(P=0.002)and WHO grade(P<0.001)had statistically significant differences in overall survival,while surgical resection method(P=0.055),radiotherapy(P=0.926),and chemotherapy(P=0.062)did not have statistically significant differences in overall survival.Conclusion Different types of posterior fossa ependyma have different clinical characteristics and prognosis.
