Gastroblastoma:report of two cases and review of literature

Abstract:Objective To explore the clinicopathological characteristic,molecular alteration,differential diagnosis,treatment and prognosis of gastroblastoma(GB).Methods The clinical characteristics,histological morphology and immunophenotype of two surgically removed GB cases were studied,and relevant literature was reviewed for a comprehensive analysis.Results At the time of diagnosis,the patients were 58 and 21 years of age.The tumors were located in the lesser curvature of gastric body and the posterior wall of antrum with the largest diameters of 2.5 and 5.5 cm,respectively.Microscopically,both tumors were composed of epithelioid and spindled cells.The epithelioid cells were arranged in sheets,nests and cords,with glandular formation in one case,while the spindled cells displayed fascicular,reticular and storiform growth patterns.Myxoid degeneration was observed in the stroma.Immunohistochemically,the tumor cells variably expressed Vimentin,CD10 and CD56,with epithelioid cells generally positive for AE1/AE3 or EMA.PTCH1::GLI2 and MALAT1::GLI1 fusions were detected in these two cases,respectively.To date,no recurrence and metastases were reported after surgery.Conclusion GB is rare,and its diagnosis relies on typical biphasic morphology,immunohistochemical features,and if necessary,molecular test.The overall prognosis of GB is favorable after surgical resection,but local recurrence or distant metastasis may occur.
