Gastric carcinoma with SMARCA4 deficiency:report of a case

Abstract:Objective To investigate the clinicopathological features of SMARCA4 (BRG1)-deficient gastric carcinoma. Methods A case of SMARCA4 (BRG1)-deficient gastric carcinoma diagnosed in Baoding First Central Hospital in 2024 was collected and analyzed in combination with relevant literature. Results The patient was a 69-year-old male, and endoscopic biopsy suggested gastric cancer. Immunophenotype: Tumor cells showed complete loss of BRG1 (SMARCA4) expression, while INI1 was positive, Syn and CD56 were weakly positive, and epithelial markers were expressed to varying degrees. LCA, CD34, CD117, DOG-1 were negative. MLH1, MSH2, MSH6, and PMS2 proteins were not deficient. p53 was wild-type, and the Ki-67 proliferation index was approximately 80%. Conclusion Current research suggests that SMARCA4 (BRG1)-deficient gastric carcinoma can be classified into three subtypes based on its morphology and immunohistochemistry. This tumor tends to occur in middle-aged and elderly males and is a highly aggressive malignant tumor, often diagnosed at an advanced stage with a poor prognosis.
