Perianal basal cell carcinoma with Paget disease:report of a case and review of literature

Abstract:Objective To study the clinical features, pathological characteristics, immunophenotype, differential diagnosis, treatment, and prognosis of perianal basal cell carcinoma (BCC) and perianal Paget's disease (PPD). Methods A case of perianal BCC combined with Paget's disease was retrospectively analyzed. Clinical data were reviewed, and histological features were studied using HE staining and immunohistochemical staining. Follow-up was conducted to evaluate the prognosis. Results A male elderly patient was admitted due to pruritus and dull pain in the perianal area, along with hematochezia for half a year. Physical examination showed a eczematous appearance of the perianal skin. Electronic colonoscopy revealed an irregular elevated mass 5 cm from the anus. Histopathological examination confirmed perianal BCC combined with Paget's disease. Immunohistochemistry showed that Paget cells expressed CK7, CDX2, Villin, and CK20, while basal cell carcinoma expressed Ber-EP4, CK5/6, and p40. Conclusion Perianal BCC combined with Paget's disease is rare. Clinically, it often presents as skin lesions, which are easily misdiagnosed, and is associated with a high probability of related internal malignancies. It should be given clinical attention.
