Clinicopathological features of a case of solid pseudopapillary tumor of the gastric parenchyma with literature review

Abstract:Objective To investigate the clinical manifestations, diagnosis, differential diagnosis, molecular pathology, and prognosis of extrapancreatic solid pseudopapillary tumor (ESPT). Methods A case of ESPT was collected, and its clinical pathological features, immunophenotype, and molecular pathology were analyzed. Relevant literature was reviewed. Results The tumor cells showed oval or spindle-shaped morphology, mainly arranged in a solid pattern, with some growing around blood vessels in a pseudopapillary manner, infiltrating into surrounding smooth muscle. The cellular atypia was minimal, and mitotic figures were rare. Immunohistochemistry showed diffuse nuclear β-catenin (+), PR (+), CD99 (membrane +), CgA (-), E-cad (-). Molecular testing revealed a mutation in exon 3 of β-catenin, without detection of KIT or PDGFRA gene mutations. Conclusion Solid pseudopapillary tumor of the pancreas (SPTP) occurring in the gastric body is very rare. Physicians with insufficient experience may lack awareness of this disease. It should be differentiated from gastrointestinal stromal tumors. The combination of immunohistochemical expression of β-catenin, PR, CD99, and molecular testing can help in the diagnosis of ESPT.
