Bilateral ovarian endometriotic cysts with Krukenberg tumor as the primary lesion of clear cell carcinoma:Clinical and pathological features

Abstract:Objective: To explore the clinical manifestations and pathological characteristics of double-sided ovarian endometrioid cysts combined with Krukenberg tumor, with clear cell carcinoma as the primary lesion. Methods: Clinical and pathological data of one case of double-sided ovarian endometrioid cysts combined with Krukenberg tumor diagnosed in the Department of Pathology of Wuhan University People's Hospital were collected, and immunohistochemistry and literature review were performed. Results: The patient was a 44-year-old female. Under the microscope, the bilateral ovarian cysts showed signet-ring cells and endometrial-like glands. A gray-white mass was found in the right adnexa. Under low magnification, the tumor cells were diffusely arranged in sheets. Under high magnification, the tumor cells appeared polygonal or boot-shaped, with abundant cytoplasm and large nuclear atypia. Many signet-ring cells were found in the lymph nodes removed during surgery. Immunohistochemistry showed that the signet-ring cells were positive for CK7, Villin, and CDX2, and negative for Pax8; the gray-white mass was positive for CK7, NapsinA, and Pax8, and negative for ER and WT-1. Gastric endoscopy biopsy showed scattered signet-ring cells in the gastric mucosa. Conclusion: Clear cell carcinoma as the primary lesion in double-sided ovarian endometrioid cysts combined with Krukenberg tumor is extremely rare. It is necessary to comprehensively analyze the patient's clinical history to avoid misdiagnosis.
